Terms of Use for
Food Allergy Consultations

Tamara Hubbard, LCPC's educational food allergy consultations provides evidence-based information, psychoeducation, ideas, suggestions, materials, and potential strategies to clients on how to manage food allergies. This information and education is not medical in nature, as this is outside of Tamara's scope of practice. This information is also not therapeutic in nature, as consultations are not counseling sessions. As such, food allergy consulting clients are not in a therapeutic relationship with Tamara Hubbard, LCPC. 
 

Clients assume all responsibility for creating and implementing their own physical, mental, and emotional well-being, decisions, choices, actions, and results. As such, Tamara Hubbard, LCPC is not and will not be liable for any action or inaction, or for any direct or indirect result of any services provided.
 

Within educational consultations, Tamara Hubbard, LCPC will not diagnose, prevent, treat, or cure medical conditions and diseases or mental disorders, nor offer nutritional, legal or therapeutic advice. The advice, ideas, suggestions, materials, and potential strategies provided to clients on how to manage food allergies are not to be used as a substitute for professional advice from medical, mental health, nutritional, legal, or other qualified professionals, and clients should seek independent professional guidance for such matters. Additionally, Tamara Hubbard, LCPC makes no warranty, either expressed or implied, as to the fitness of any recommendation, product, or service  as a result of this agreement.

 

Educational food allergy consultations take place virtually via the HIPAA-compliant electronic health records system, Simple Practice. Simple Practice will autogenerate an emailed appointment reminder 48 hours prior to your session with the video link. 

Educational food allergy consultations are not covered by insurance, and therefore, consultation clients are required to place a credit or debit card on file within Simple Practice's HIPAA-compliant client portal. The credit or debit card on file will be used to process payment for the food allergy consultation session within 24 hours of the appointment. Please be assured that your credit or debit card information is stored in your encrypted client account on Simple Practice, and will only be charged for your consultation session(s). (Please note: Food allergy consultation appointments will be rescheduled or cancelled for clients who do not have or are unwilling to place a credit or debit card on file prior to their appointment). 

Your consultation session appointment is scheduled for your benefit, and therefore, you will still be required to pay for the session if you missed or skipped it. If you know in advance that you need to cancel or reschedule your consultation session, please let Tamara know at least 48 hours prior to your appointment to avoid being charged. 


Please note that if during the consultation session it's determined that the scope of work is better suited for therapy, Tamara will let you know. If the consultation session ends early due to that determination, the session rate will be prorated based on the amount of time spent consulting/talking.

By engaging in educational consultation sessions, you are agreeing to these terms of use.
